Step 1: Complete the Surgical Readiness Questionnaire

Please note: This questionnaire is extensive and asks deeply personal questions but all of the information is necessary and will be used to complete a comprehensive 4-page report to provide to your surgical team, doctors, and health insurance provider.

Step 2: Schedule your appointment with Dr. Shapiro

Once the form is submitted, schedule a one-time, FREE consultation with Dr. Lauren Ashley Shapiro.

Step 3: Meet with Dr. Shapiro

During your appointment with Dr. Shapiro, you’ll review your questionnaire, discuss your needs, and have the opportunity to ask any questions about the process.

Step 4: Receive Your Report

Within 2-3 weeks, Dr. Shapiro will prepare your surgical readiness letter and upload it to her secure client portal. From there, you can easily access and share it with your surgical team, doctors, or health insurance provider.
